Puppy purchaser questionnaire
The dogs that I am breeding have all their health clearances, as well as what I feel
to be of "proper temperament". I actively show my dogs in conformation and have
done so for over 10 years.
Unless a puppy is sold as a show prospect, all buyers agree to a spay/neuter contract and
with AKC Limited registration and agree not to register any of these puppies with any other
registry other than the American Kennel Club. The AKC Limited Registration stipulates
that no offspring of this dog can be registered with the AKC.
Puppies will have tails docked and dew claws removed at about3 days old. The
puppies will be examined by my personal veterinarian and have been started on
their vaccinations and thoroughly checked. They will also have their eyes
examined at 6 weeks of age by an ACVO veterinarian, as well as being tested for
genetic PRA status (unless both the sire and dam are genetically PRA clear)
Please note that puppies will not be placed before 8 - 10 weeks of age.
References from previous buyers are gladly provided.

Please tell me your previous dog ownership history, what breed/breeds you have
owned, and if the dogs did not live with you their entire life, where did they go?

available, would you consider something else?
